How does it work?
Success Link will need a referring partner to complete a referral form to enable us to understand the needs of the person for whom accommodation is required for, this will allow us to determine individual eligibility for our service
We also need to understand the following
Type/How much care does the person require?
Features does the property need to suit tenant need?
What services need to be provided
(i.e. furniture/equipment, gardening, communal cleaning.)
Can we offered accommodation at an appropriate rent level?
We will conduct financial/affordability assessment?
Do current accommodation in our portfolio suits the need of tenant?
We conduct a need assessment to confirm tenant service eligibility?
We may wish to know why alternatives accommodation considered to be unsuitable?
How long will the accommodation be required for long or short-term?
Is there any deadline, and what factors affect the deadline?
